BSA Medical forms are required for all scouts.
Outings of 72hrs or less:
All scouts and adult leaders who attend outings 72hrs or less must have complete and valid BSA medical forms, Parts A&B plus front/back copy of insurance card on file with the troop.
Outings 72hrs or more:
All scouts and adult leaders who attend outings longer than 72hrs must have complete and valid BSA medical forms, Parts A, B & C (this part must be signed by a physician) plus front/back copy of insurance card on file with the troop.

In order to attend summer camp, it is required that complete medical forms are available for every participant. PLEASE NOTE: No BSA camp will allow a scout or adult leader attend without complete forms and will be turned away. Additionally, no scout or adult leader will be allowed to attend a high adventure outing without complete forms on file.
All scouts attending to summer camp must also fill out additional forms to assist the adult leaders in managing medications during camp. Detailed information will be provided by the camp coordinators closer to departure date.
